High percentage of US patients on placebos without knowing it

A new study amongst doctors in the United States on the use of placebos—pills with no medical effect—shows that almost half of the questioned practitioners prescribe placebos, most of them within the last year.

The majority of 466 faculty physicians at Chicago-area medical schools interviewed by a research group of the University of Chicago stated that placebos are useful to calm a patient down or to respond to demands for medication that the doctor disagrees with, i.e. “to get the patient to stop complaining”.

96 percent of the physicians surveyed believe that placebos can have therapeutic effects. Close to 40 percent stated that placebos could benefit patents physiologically as well as mentally.

Twelve percent of surveyed physicians think that placebos should be banned from clinical practice. Among the doctors who prescribed them, one in five said they outright lied to patients by claiming a placebo was medication. But more often the physicians came up with ways to explain like that “this may help you but I’m not sure how it works.”

The American Medical Association (AMA), the largest association of U.S. doctors and medical students, tells its members that “[p]hysicians may use placebos for diagnosis or treatment only if the patient is informed of and agrees to its use.” The research, published in Journal of General Internal Medicine this week, is the first major U.S. study of doctors on the use of placebos since 1979.

France out-wit Spain 3-1 and clinch quarter-final spot

France came from a goal down to win their round of sixteen game 3-1 against Spain in Hanover, Tuesday.

The win was a tactical triumph for the French coach Raymond Domenech whose team gave up 61 per cent of ball possession yet restricted their opponents to seven shots – two on target – and ran out three goal winners.

Nor did the French have many chances as for the majority of the game Spain patiently passed the ball around them. When they did have the ball they constantly pried at the Spanish offside line and therein found the weakness that lead to victory.

Though the French were offside eight times the crucial equaliser came from a Franck Ribéry run on to a Patrick Viera pass that beat the line of Spaniards close to half time. At speed Ribéry rounded Iker Casillas to slot home.

So solid were the French in defence intercepting passes, blocking shots and making tackles the only goal conceded – which put them behind – was from a penalty kick. This was slammed in low by David Villa on 27 minutes after Pablo had been clattered from behind by Liliam Thuram.

Ten minutes into the second half the score was at 1-1 but the French continued to test the Spanish offside trap. A worried Luis Aragonés brought on two substitutes and changed his system from 4-3-3 to 4-2-3-1. The Spanish defence sat closer to their penalty area so there was less room to pass balls behind them.

But still the Spanish line was caught on 82 minutes when Thierry Henry was fouled by Puyol as he attempted to connect with another through ball. Henry fell as if hit in the face by Puyol although this could not be confirmed with television evidence. Zinedine Zidane floated the resultant free kick into the box, there was an inadvertent flick on from a Spanish player and Viera nodded in at the far post off Sergio Ramos.

As the Spanish pressed for a last minute score the French broke through again, Zidane this time cut back to lose Puyol, and with the keeper off balance he reversed his shot to the near post to put the game out of reach.

So efficient was the French performance FIFA statisticians logged the team with just 19 minutes actual playing time. Brazil, who the French will play in the quarter-finals, too conceded the majority of possession in their game but in comparison beat Ghana 3-0 with 27 minutes actual playing time, Tuesday.

Emergency spacewalks planned to fix International Space Station

NASA is scrambling to solidify plans to fix the broken down cooling systems on the International Space Station during two spacewalks, currently planned for Friday and Monday. The systems broke down late Saturday July 31, triggering alarms that woke the six astronauts (three Russian and three American) currently onboard the station. NASA’s flight controller said the astronauts are not in any danger, but that science experiments are on hold until the problem can be fixed.

The first spacewalk is set to begin Friday at 6:55 a.m. EDT, NASA officials said. The two planned spacewalks will be conducted by astronauts Douglas Wheelock and Tracy Caldwell Dyson. They will replace an ammonia pump module, which shut down Saturday. The pump’s failure prompted the astronauts to shut down several other systems as well. The spare pump had been delivered to the station in July 2006, during STS-121.

Normally, major repair spacewalks take weeks of preparation. An unrelated spacewalk had already been planned for this Thursday, which would have been conducted by Wheelock and Dyson, so the space suits and equipment are ready to go. NASA decided late Monday evening that the spacewalk would be postponed to Friday to allow for more preparation.

In October 2010 the space station is expected to break the record for the longest continuously inhabited space station, a record currently held by Mir.

Surgeons reattach boy’s three severed limbs

Tuesday, March 29, 2005A team of Australian surgeons yesterday reattached both hands and one foot to 10-year-old Perth boy, Terry Vo, after a brick wall which collapsed during a game of basketball fell on him, severing the limbs. The wall gave way while Terry performed a slam-dunk, during a game at a friend’s birthday party.

The boy was today awake and smiling, still in some pain but in good spirits and expected to make a full recovery, according to plastic surgeon, Mr Robert Love.

“What we have is parts that are very much alive so the reattached limbs are certainly pink, well perfused and are indeed moving,” Mr Love told reporters today.

“The fact that he is moving his fingers, and of course when he wakes up he will move both fingers and toes, is not a surprise,” Mr Love had said yesterday.

“The question is more the sensory return that he will get in the hand itself and the fine movements he will have in the fingers and the toes, and that will come with time, hopefully. We will assess that over the next 18 months to two years.

“I’m sure that he’ll enjoy a game of basketball in the future.”

The weight and force of the collapse, and the sharp brick edges, resulted in the three limbs being cut through about 7cm above the wrists and ankle.

Terry’s father Tan said of his only child, the injuries were terrible, “I was scared to look at him, a horrible thing.”

The hands and foot were placed in an ice-filled Esky and rushed to hospital with the boy, where three teams of medical experts were assembled, and he was given a blood transfusion after experiencing massive blood loss. Eight hours of complex micro-surgery on Saturday night were followed by a further two hours of skin grafts yesterday.

“What he will lose because it was such a large zone of traumatised skin and muscle and so on, he will lose some of the skin so he’ll certainly require lots of further surgery regardless of whether the skin survives,” said Mr Love said today.

The boy was kept unconscious under anaesthetic between the two procedures. In an interview yesterday, Mr Love explained why:

“He could have actually been woken up the next day. Because we were intending to take him back to theatre for a second look, to look at the traumatised skin flaps, to close more of his wounds and to do split skin grafting, it was felt the best thing to do would be to keep him stable and to keep him anaesthetised.”

Professor Wayne Morrison, director of the respected Bernard O’Brien Institute of Microsurgery and head of plastic and hand surgery at Melbourne’s St Vincent’s Hospital, said he believed the operation to be a world first.

Study tests the use of Google as an aid for medical diagnoses

In an internet-based study published today on the website of the British Medical Journal, Australian investigators tested how well Googling could help to establish a difficult diagnosis. The search results met against their criteria in 15 out of 26 cases.

Previous anecdotes of the use of Google in reaching a difficult diagnosis are presented in the paper. In a case described in the New England Journal of Medicine (NEJM), a physician diagnosed IPEX syndrome after submitting the typical diagnostic clues to the search engine. A patient’s father replied to doctor’s saying that his son had acute subclavian vein thrombosis of unknown origin: “But of course hehas Paget-von Schrötter syndrome.”

Hangwi Tang and Jennifer Hwee Kwoon Ng from the Princess Alexandra Hospital in Brisbane selected clinical cases from the NEJM blind to the correct diagnosis. They selected 3 to 5 rather specific features, searched Google, and selected the 3 disorders from the results that seemed to fit the case best. If one of those was right, Google was said to be “right”.

The scientists admit that an internet search probably works better with specific symptoms rather than with phrases such as “fever ill pain”. Also, the percentage may depend on the operating physician’s knowledge base, and the test was not double-blind.

Experts commented that the internet is not a replacement for doctors, but should be seen as a way of supporting doctors and patients. A spokeswoman for the Patient Association added that “a lot of sites are not credible. There are lots of good sites out there, but we also know that there are many that are not credible.”

Large increase in number of foreclosures across US

Recently released figures have revealed that the number of foreclosures in California, United States has more than quadrupled. The figures for the first quarter of 2008 show a significant increase on previous figures.

There were 47,171 foreclosures on California properties in the first quarter of 2008. The figure for the same period last year was just over 10,000.

The Attorney General of Arizona released a report that showed an increase. It said that “the collective efforts of servicers and government officials to date have not translated into meaningful improvement in foreclosure prevention outcomes.” The Attorney General also commented on the report:

We continue to see a rising number of foreclosures in Arizona, which is a significant drag on Arizona’s economy. This report confirms that efforts made by servicers to prevent unnecessary foreclosures are not enough. We need to explore additional approaches to prevent tens of thousands of unnecessary foreclosures in Arizona.

Mercifully, San Diego does not have rent control. Unfortunately, San Diego does have its close relative-the unwelcome mother-in-law of rent control-better known as eviction control or for cause eviction.

San Diegos eviction control ordinance substantially impacts a landlords right to end a month-to-month tenancy. Moreover, the ordinance can be a trap for the unwary landlord unfamiliar with its mandatory notice provisions. A notice terminating a month-to-month tenancy that does not comply with the eviction control ordinance could cause the inexperienced landlord to lose his eviction case at trial and have to start the eviction process all over again. What is San Diegos eviction control ordinance? The ordinance applies to every tenancy of two years or greater duration. When a tenant has lived in your rental property for two years or more, you may only terminate his month-to-month tenancy, or refuse to renew his lease, for certain specified reasons. On the face of it, the authorized reasons in the ordinance dont seem unreasonable. As examples, a landlord may only terminate the tenancy for things such as nonpayment of rent, breach of lease, tenant use of the property for an illegal purpose, or landlord withdrawal of the premises from the rental market just to name a few. The ordinances restrictions wouldnt appear too onerous to most landlords since a landlords typical reasons for terminating a tenancy match those permitted by the eviction control ordinance. However, the ordinances mandates do not end there. The ordinance further requires that the landlord serve on the tenant a notice setting out the reason or reasons for terminating the tenancy at the same time that the landlord serves the 30-day notice, 60-day notice, or 3-day notice, whichever the case requires. In other words, to comply with San Diegos eviction control ordinance, you must first have a permitted basis for terminating the tenancy. In addition, you must serve a separate notice on the tenant setting forth that permitted reason at the same time that you serve the notice terminating the tenancy. Thus, a tenant who is protected by the eviction control law should get two notices, one terminating the tenancy (the 3-day, 30-day, or 60-day depending upon the case) plus another notice setting forth the reason authorized by the eviction control ordinance. Additionally, to be valid, both notices must be served according to law, i.e. they must be personally served on the tenant or served through a valid substitute service or post and mail service. I cant tell you how many landlords I have seen in my practice make the mistake of not complying with the eviction control ordinance. In fact, nearly every client I see who has tried to terminate a month-to-month tenancy with a 30-day or 60-day notice either didnt comply with the ordinance or didnt serve it right. The consequences for the landlord to not terminating the tenancy right can be dire. At a minimum, the landlord will suffer the lost time and inconvenience of having to serve a new notice. If the landlord has sued on a bad notice, the consequences can be worse. The tenant will win the eviction case and the landlord will have to start the entire process over again, thus losing more time, rent, plus costs and attorneys fees. If you want to end a month-to-month tenancy, or not renew a tenancy, on some ground other than non-payment of rent, you should consult a lawyer to make sure that you comply with San Diegos eviction control ordinance.
